you prepare for the 2010-2011
school year, please make
sure your registration
forms include the following:
1.
State
Student ID (SSID) Fields:
Student
first name, last
name, middle
name,
gender,
and date of birth are
required fields. Please
use the student's birth
certificate to enter
all of
these fields accurately
as you enroll the student.
All
five of these fields
are used by the State
Student ID (SSID) database
to check for a match.
Do not skip the middle
name or enter a middle
initial. A mismatch will
create inaccurate student
reports across the years.
Schools will not receive
progress scores for the
UPASS report.

Ethnicity and Race Fields:
Federal
regulations as

school
districts and states to
follow new
standards in collecting
individual-level
ethnicity
and race
data by the 2010-2011

A
two-part question must
be used to collect data
about students’ or
staff’s
race and ethnicity,
with the ethnicity part
asked first.
