After a charter school is approved by USOE, there are several forms that must be filled out to grant access to essential processes within USOE. The webpages listed below require a password. Contacts and request forms are listed for each of these.

Assessment: Preprints, Downloads of raw scores and profiles https://cs.schools.utah.gov/assessment/

USOE pre-prints student names on answer documents based on the data uploaded to this webpage. Results for assessments such as CRT, DWA, UALPA are available for download from here.

Here is an overview of the pre-print process.

Move-IT

USOE provides secure file transfer via Move-IT. A variety of departments within USOE use this system to share student information with schools. You will be given one user ID and password for Move-IT.

UTOS

Utah Test Ordering System (UTOS) is used to order testing materials. Contact Assessment at USOE 538-7500 for access.

UCA (formerly CUSAP)

Utah Consolidated Application (UCA) replaces the Consolidated Utah Student Achievement Plan (CUSAP). This is part of the No Child Left Behind program. All charters must fill out a UCA plan, whether or not they are applying for any Title Funding.

School LAND Trust (Trustlands)

SSID

https://cs.schools.utah.gov/SSID/

The Statewide Student ID (SSID)is a unique number that follows a student as they enroll at different schools. It connects data across schools and districts.
